Vital Statistics

To obtain a certified copy of a vital record such as a Birth, Death or Marriage the applicant must complete an application and provide photo identification with current address listed on the same or two other forms of identification as follows:

• Vehicle Registration
• Insurance Card
• Voter Registration
• Passport
• Green Card
• County Photo Identification
• Utility Bill(s)
• Photo Identification Discharge From Jail
• Police Report
• Fire Report

Those who may request a certified copy:

• Subject
• Parent
• Legal Guardian (must submit proof)
• Legal Representation (must submit proof)
• Subject's Spouse
• Subject's Grandchild
• Subject's Sibling
• Commissioner
